What is Chronic Disease Management?

Chronic Disease Management (CDM) refers to the ongoing, long-term care of conditions that cannot be cured but can be effectively managed to prevent complications and maintain quality of life.

At Mission (Hougang) Medical Clinic, our doctors work closely with you to develop a personalised care plan — monitoring your condition, adjusting medications as needed, and supporting you with lifestyle guidance to keep your numbers under control.

We are enrolled in the Chronic Disease Management Programme (CDMP), which allows eligible patients to use MediSave to pay for their chronic disease treatment — reducing out-of-pocket costs significantly.

We develop a tailored plan covering medications, lifestyle changes, monitoring targets, and follow-up schedule to keep your condition under control.

Regular review & monitoring

Scheduled follow-ups to review your progress, adjust medications, order lab tests, and ensure your condition is well managed over time.

Chronic disease consultations are subsidised under CHAS, Healthier SG, Pioneer Generation and Merdeka Generation schemes. Eligible patients can also use MediSave via the CDMP to offset treatment costs. Learn more →

Yes — as a CHAS-accredited and Healthier SG-enrolled clinic, we can provide the same chronic disease care as a polyclinic, often with shorter waiting times and a more consistent patient-doctor relationship.
Yes — under the Chronic Disease Management Programme (CDMP), patients enrolled in Healthier SG can use MediSave to pay the full remaining bill after CHAS subsidies, with no cash co-payment required.
This depends on your condition and how well it is controlled. Most patients come in every 3 to 6 months for routine review and lab tests. Your doctor will advise you on the appropriate frequency for your situation.
Your doctor will adjust your medication or treatment plan and may refer you to a specialist if needed. We work closely with restructured hospitals and specialist outpatient clinics to ensure you get the right level of care.
Yes — if you are enrolled in Healthier SG with MMC as your preferred clinic, you can access enhanced subsidies under the Healthier SG Chronic Tier, including medication subsidies of up to 87.5% for CHAS cardholders.
